Medical Exemptions for the COVID Vaccine

There are only four reasons that LFM providers will agree to write a medical exemption for a patient to avoid getting the COVID vaccine:

  1. History of anaphylactic reaction (treated in the emergency room or inpatient) to a vaccine in the past
  2. History of Guillain-Barre syndrome after a vaccine in the past
  3. It has been less than 10 days from the start of symptoms (or positive test if asymptomatic) of an active COVID infection
  4. Having had a monoclonal antibody infusion in the last 90 days

If you believe that you have extenuating conditions that are reasonable, please call to schedule an appointment (phone, video or in-person) with your provider to further discuss.
