Insurance

Lakewood Family Medicine participates with several insurance companies, and we are committed to helping you receive payment for services covered by your plan. We recommend calling our office to verify our participation with your insurance, as the list of participating carriers/networks may vary.

Your insurance’s participation status does not necessarily mean that Lakewood Family Medicine services performed will be covered by your insurance carrier/network. This is decided by the coverage that you or your employer purchase. To confirm coverage for any specific services, please contact your insurance carrier/network.

Billing

We require payment at the time of service and accept cash, check, VISA, MasterCard and Discover.

Co-payments and any prior family balance due will be requested at the time of registration. If you or your family have payment concerns at any time, please let us know.

Financial Assistance Options

Lakewood Family Medicine is willing to offer payment plan options to patients with large balances or difficult financial circumstances. Please call (616) 396-6516 and a billing representative will be happy to review payment options that may be available to you.
