Lakewood Family Medicine Physicians are board certified by the American Board of Family Practice. They are members of the Holland Hospital medical staff, The American Academy of Family Physicians, the Michigan State Medical Society, the Lakeshore Physicians’ Organization (LPO), and the Holland Area Physician Hospital Organization. (PHO). The team has several members that contribute to various Hospital committees.
|Timothy Smith, M.D.
Dr. Smith is a graduate of the University of Michigan and the University of Michigan Medical School. He completed the Grand Rapids Area Medical Education Center (GRAMEC) Family Practice Residency. Dr. Smith and wife Lynn have two sons.
|Eric K. Gustafson, M.D.
Dr. Gustafson is a graduate of Hope College and Michigan State University College of Human Medicine. He completed the Grand Rapids Family Practice Residency. Dr. Gustafson and wife Linda have two sons and two daughters.
|Charlotte Johnson, M.D.
Dr. Johnson is a graduate of Hope College and MSU College of Human Medicine. She completed the Grand Rapids Family Practice Residency in 1996 and has been in practice at Lakewood Family Medicine since then. She values the compassionate staff and progressive attitudes of the practice. Dr. Johnson works part time to better balance time with her husband and three children. Hobbies that she enjoys are knitting, reading cooking magazines, and baking.
|Dr. Kurt A. Lindberg, M.D.
Dr. Lindberg grew up in Farmington Hills, Michigan and attended the University of Michigan where he studied Aerospace Engineering. While working at McDonnell Aircraft in St. Louis, his experiences with mission and charity organizations began to draw him to the helping professions. He subsequently attended medical school at the University of Michigan, during which he was married to his wife, Kendra. Upon graduation, they accepted a position in the Boston University-affiliated residency program in Beverly, MA and came to join Lakewood Family Medicine in 1999. The Lindberg’s have three children and enjoy spending time with family and friends. Dr. Lindberg serves as the President of the Holland PHO (physician/hospital organization), is a member of the Pisacano Foundation (a leadership development program in Family Medicine), and writes exam questions for the American Board of Family Medicine.
|Jon W. Kroeze, M.D.
Dr. Kroeze graduated from Calvin College, Wayne State University Medical School, and completed his training at the Grand Rapids Family Practice Residency program. He moved to Holland and joined Lakewood Family Medicine in 1994. Dr. Kroeze and his wife Kathy are blessed with three children-Kari, Danny and Sara. Kathy is a substitute teacher and enjoys working at their children’s school. The Kroeze’s enjoy sports as a family, their favorites are; playing tennis and downhill skiing. Dr. Kroeze enjoys listening to music and singing on the praise team at church.
|Patrick Hulst, M.D.
Dr. Hulst is a graduate of Calvin College and Wayne State University School of Medicine. He completed the York Hospital Family Practice Residency in York, Pennsylvania. Dr. Hulst and wife Jan have one son and two daughters. He has a passion for Corvettes, and started the Lakeshore Corvette Club in Holland.
|Cecilia DeCook, M.D.
Dr. DeCook has been practicing at LFM since 1998. She enjoys the full range of family practice with a special interest in Women’s Health. She is a graduate of Michigan State University, and the University of Michigan Medical School, earning her MD in 1988. She completed her residency in Family Practice at Providence Hospital in Southfield, Michigan. She keeps up with a busy family life outside the office with three children all active in school and extra-curricular. Her husband also practices here in Holland as a general surgeon. When Dr. DeCook is not running after patients or children, she enjoys teaching Sunday School, reading, and all kinds of outdoor activities, especially tandem biking with her husband. The thing she enjoys most about working at LFM is the spirit of teamwork to provide the best of care for our patients.
|Martin Berent, M.D.
Dr. Berent received his Bachelor of Science degree from Wayne State University and his Doctor of Medicine from Wayne State University School of Medicine. He graduated with clinical honors in Family Practice. Dr. Berent and his wife Farah have two sons. He enjoys varied interest including Polish folk dancing, volleyball, basketball, and architecture/design.
|Beth Peter, M.D.
Dr. Peter is a graduate of the University of Minnesota Medical School and completed her internship and family medicine residency at Poudre Valley Health System in Fort Collins, Colorado. She and her husband have three children. Together they enjoy gardening, winter sports, and music.
|Jill K. Miedema, M.D.
Dr. Miedema joined LFM in 2012. She enjoys being a family doctor because of the chance to form long-term relationships with patients and their families, and because of the focus on preventive health. She is a graduate of Calvin College and Michigan State University’s College of Human Medicine, earning her MD in 2005. She completed her residency in Family Medicine at Sparrow Hospital in Lansing, Michigan. She was a National Health Service Corps Scholar and spent four years working in a community health center in inner city Minneapolis. She speaks Spanish and has spent time abroad in Spain and Ecuador. Dr. Miedema and her husband Chris have two daughters and one son. The thing she appreciates most about LFM is the commitment to quality care and genuine compassion and dedication of the staff.
|Dr. Robert Bruxvoort, MD
Dr. Bruxvoort is a graduate of the University of Northern Iowa and the University of Iowa College of Medicine. He completed his residency in 2004 at the Lincoln Medical Education Partnership in Lincoln, Nebraska. He practiced in Pella, Iowa for nearly ten years before joining Lakewood Family Medicine in 2014. Dr. Bruxvoort and his wife, Alicia, have two sons and three daughters.
