Things to know before your visit:
Insurance
We participate with several insurance companies and are committed to assisting you in receiving payment for covered services under you plan. (Insurance Carriers) We suggest that you call our office to verify that we participate with your insurance, as the list may vary from time to time. The participation status does not imply that the services performed will be covered in whole or part by your insurance carrier/network. This is decided by the coverage that is purchased by your employer. To confirm coverage for your services, please contact your insurance carrier/network. If you do not see your health plan and/or network on this list, call us at (616) 396-6517.
Payment
We require payment at the time of service. We accept cash, check, VISA, Master Card and Discover. If you are responsible for a co-payment we will request that at the time of registration. If this presents a hardship to you and/or your family at any time, please feel free to discuss this with us. (Office Payment Policy)
Financial Assistance Options
We are willing to offer payment plan options to patients with large balances or difficult financial circumstances. We also offer assistance to patients who do not have insurance coverage and meet certain criteria based on Federal Poverty Income Guidelines. Please call us at (616) 396-6517 and a billing staff representative will be happy to review payment and financial assistance options that may be available to you.
