Telephone Nurse Advice

Our office has a trained nurse staff to give advice on many common medical problems over the telephone. During your telephone interview, the nurse may advise you to come in for a visit, if your symptoms warrant this, or she may need to send a message to a physician for advice and get back to you. This service can be accessed by calling direct at (616) 396-6771 or by calling our main phone number (616) 396-6516 and the operator can transfer your call. 

 

Injection Clinic

We have an injection clinic that is open daily for a variety of services.
Please call our scheduling department at (616) 396-6568 to see if you need an
appointment or if you can walk in at your convenience for your desired service.

  • Monday: 8:00am – 11:45am, 1:00pm – 4:45pm
  • Tuesday: 9:00am – 11:45am, 1:00pm – 5:45pm
  • Wednesday, Thursday, Friday: 8:00am – 11:45am, 1:00pm – 4:45pm

Care Manager – Beth Venema R.N.

Beth is a graduate of the Blodgett Memorial Medical Center School of Nursing.  She has previously worked in oncology, geriatrics, and pediatrics.  Beth also has experience with insurance utilization review, and most recently in home care case management and coordinating the care during the transition from hospital to home.

Beth is new to Lakewood Family Medicine and is working with our health care team to deliver additional care that supports our patients living with chronic diseases.

She is married to Dale who is an electrical engineer, and has one son and two daughters.  When she is not following children to sporting events, she enjoys walking, cooking, and reading books.

Off Site Test Scheduling and Specialist Appointments

The Out Patient Scheduling Department will help facilitate any referral
to a specialist. They will assist with scheduling any special testing, laboratory
testing, mammograms, etc. Our direct number for this service is (616) 396-6807.

Language Interpretation Service

Language interpretation is available for those who need assistance understanding any of our
services due to language barriers.
