Our Facility
Lakewood Family Medicine is located on Holland’s North side, on 120th Ave between James Street and Lakewood Blvd. Our building provides a comfortable and spacious environment with a separate children’s waiting area. Laboratory services provided by Holland Hospital are conveniently located on site.
MAX transportation service is available to our facility several times daily.
